BAPTISM PREPARATION
 | We wish to enroll in the sacramental preparation program for our child's
baptism.

| We understand that we are to contact Earl Roberts, pastoral assistant at the parish office (832-2262) upon submitting this registration form to arrange for a meeting at the parish office center to begin the preparation process. | ||
| A $35 sacramental preparation fee will be presented at the time of our office meeting. (This covers cost of the books and materials provided to you.) Please make checks payable to: St. John Neumann Church (memo: Baptism Preparation) If this poses a hardship, fee can be waived! | ||
| We commit ourselves to read the materials provided prior to attending the Baptism Program. | ||
| We understand that we may invite godparents to attend the program, but that this is not mandatory. | ||
| We understand that for reasons of comfort, baptism preparation ideally is to be undertaken during the middle trimester of pregnancy; in the event our child has already been born, we will arrange for child care at home so that our attention may be devoted to the program. |
